Please Include the following documentation:

1. Last two years tax returns signed with all schedules attached

2. Last two years W-2's and/or 1099's

3. Last two pay stubs

Assets-for this section please include: Name, address account # and account balance; include copies of last three months statements

1. Checking accounts

2. Savings Accounts

3. Stocks or Bonds owned ( Company name/number & description)

4. 401k, IRA, Keogh and Pension plans ( Company name/number & description)

Liabilities-for this section please include: Name, address account # and account balance

1. Mortgages, Loans & Credit Cards

If you are Self-Employed or have a 25% or greater interest in a corporation or a partnership please include the following documents.

1. Last two years business tax returns

If you are refinancing please include:

1. Last twelve months canceled mortgage checks, front and back or a Mortgage statement with escrow analysis or paid tax and insurance bills for the current year.

2. Copy of your Deed.

If you are purchasing please include:

1. Copy of the signed Agreement of Sale
